To the prospective student,
Do you think you are compassionate, have an interest in healthcare, and want to work with the elderly and the disabled? If you answered YES, this is the right place for you… It is advised that you thoroughly review the information that has been provided before completing the registration form, in order to choose the program that is right for you. Then, you can either download and complete the registration form or you can also register online. Please note the admission requirements below: General Eligibility for Admission Requirements - Age Requirements:
- Caregiver -- at least 18 years old - Asst Caregiver -- not less than 16 years old - Must possess a valid photo ID
- No criminal conviction within last 5 years
- Be able to lift 50 lbs
- Must pass physical exam: TB Test
- Should be able to speak and read in English
- GE 7.0 or better
For Manager’s Training: - Must be a certified Caregiver prior to taking Manager’s Training Course
- R9-10-724.B.4 Accept documentation that an individual is certified as a nursing assistant under A.R.S. Title 32, Chapter 15 as verification of the skills and knowledge required in subsection (B)(3)(b)(i) and (ii);
Nursing Assistant Training Program: - Minimum age requirement: 17 years old
- Be accompanied by a parent or guardian if under 18 yrs old
- Drug Testing, CPR and First Aid Training , TB testing or chest X-ray results within the last year, Fingerprinting
- Citizenship status documentation (please see next page)
- Complete Application for Enrollment questionnaire
- Complete Receipt of Student Packet (Policies and Procedures)
- Felony Bar within the last five years
HEPATITIS B AND TB TESTS: If the student does not have a current TB test (less than one year), this training program will provide one. For a student who has a positive TB test, a physician’s statement (by chest x-ray) required.
