Supervisory Care Services: - 20 hours of training, or the amount of time needed to verify that an individual demonstrates the specific skills and knowledge in the learning objectives in the components prescribed by the AZDHS.

Certificate of Completion in Assisted Living – “Supervisory Care Services” – total of 20 hours
Personal Care Services: - Should have completed and passed Supervisory Care Services
- 30 hours of training, or the amount of time needed to verify that an individual demonstrates the specific skills and knowledge in the learning objectives in the components prescribed by the AZDHS.
- Should pass 70% of exam and pass skills test
Certificate of Completion in Assisted Living – “Personal Care Services” – total of 30 hours
Directed Care Services: - Should have completed and passed Supervisory Care and Personal Care Services
- 12 hours of training, or the amount of time needed to verify that an individual demonstrates the specific skills and knowledge in the learning objectives in the components prescribed by the AZDHS.
- Should pass 70% of exam and pass skills test
Certificate of Completion in Assisted Living – “Directed Care Services” – total of 12 hours
Manager Training - Should have been certified as a caregiver
- 8 hours of training, or the amount of time needed to verify that an individual demonstrates the specific skills and knowledge in the learning objectives in the components prescribed by the AZDHS.
- Should pass 70% exam
Certificate of Completion in Assisted Living – “Manager’s Training Course” – total of 8 hours - After certification, must also apply with the Arizona Board of Examiners for Managers of Assisted Living Facilities, and should pass the State Board exam to be able to work as Manager of an assisted living facility.
Requirements for certification as a manager of an assisted living facility: - High school diploma or G.E.D.
- 2080 hours of paid work experience in a health related field within the last 5 years
- Completed DHS approved training courses (personal, supervisory, directed, and manager)
- Pass the Board examination
- Current CPR and Basic First Aid training
- Submit all applicable information required under R4.33.403 - www.nciabd.state.az.us
Nursing Assistant Training Program 120-hour course providing the CNA student, knowledge and skills necessary to meet the demands of elderly nursing home residents, ultimately leading to improvements in patient care and worker satisfaction. - Minimum age requirement: 17 years old
- Be accompanied by a parent or guardian if under 18 yrs old
- Drug Testing, CPR and First Aid Training , TB testing or chest X-ray results within the last year, Fingerprinting
- Citizenship status documentation (please see next page)
- Complete Application for Enrollment questionnaire
- Complete Receipt of Student Packet (Policies and Procedures)
- Felony Bar within the last five years
HEPATITIS B AND TB TESTS: If the student does not have a current TB test (less than one year), this training program will provide one. For a student who has a positive TB test, a physician’s statement (by chest x-ray) required.
